New online referral form

In response to feedback from health professionals, FlexEquip has reviewed and relaunched the FlexEquip online referral form.

To make the form more usable for the health professionals a number of changes have been made, including a new feature that enables the referring health professional to ‘save’ their service provider details to auto fill with the next referral.
